One particularly interesting clue is how cold the infected become before turning.

Normally, maintaining a stable body temperature depends heavily on the hypothalamus, autonomic nervous system, blood vessels, endocrine system, and skeletal muscles all working together. If an infection began interfering with hypothalamic regulation while simultaneously triggering extreme peripheral vasoconstriction, the skin could become noticeably cold even while the body's internal systems were entering complete chaos.

Then there's the ridiculous physical performance.

The infected in This Is Not a Test can sprint aggressively and continue pursuing people with seemingly little concern for exhaustion, injury, or self-preservation. That could point toward massive sympathetic nervous system activation involving adrenaline, noradrenaline, increased cardiac output, redirected blood flow, suppressed pain signaling, and removal of the normal neurological limits that keep humans from destroying their own bodies.

But perhaps the strangest part is that the original person may not be completely gone.

When Sloane encounters someone important to her late in the movie, there appears to be a brief moment where recognition interrupts the attack.

That raises an interesting neurological possibility.

What if the infection doesn't erase the brain?

What if it simply overwhelms it?

We’re going to look at the amygdala, hypothalamus, limbic system, prefrontal cortex, autonomic nervous system, adrenal response, pain pathways, temperature regulation, and motor control to figure out what kind of pathogen could theoretically turn a normal person into one of these things.

And more importantly:

Would the person still be trapped somewhere inside?
